Each immunoglobulin molecule contains either two kappa or two lambda light chains.
The ratio of kappa to lambda light chains is measured to determine the clonality of B-cell proliferation, which aids in the differentiation of reactive infectious vs. malignant
B-cell proliferation.
 
 
Benign reactive proliferations
Every infection contains a benign or reactive B-cell response. The kappa/lambda ratio is polyclonal with a physiological admixture of kappa/lambda of approximately 2:1.
 
 
Malignancies
B-cell malignancies usually express a single light chain, or monoclonality, as determined by a marked predominance of either a kappa light chain population or lambda light chain population.
